  • Assessments, plans, and notes that support your recovery or growth

  • Optional holistic or wellness-related information you choose to share

You are always encouraged to ask questions about what information is collected and why.

How Your Information Is Used

Your information is used solely for purposes permitted by HIPAA and 42 CFR Part 2, including:

  • Supporting recovery, peer support, coaching, or holistic services

  • Developing individualized plans and goals

  • Coordinating care with other providers only with your written consent

  • Processing insurance or billing matters when applicable and authorized

  • Quality assurance, supervision, or compliance activities as required

Your information is never used for marketing, research, or non-care purposes without your explicit authorization.

Enhanced Confidentiality Under 42 CFR Part 2

Because Paraclete Recovery supports individuals affected by substance use, your records are protected under 42 CFR Part 2, which offers additional privacy safeguards beyond HIPAA.

This means:

  • I cannot confirm or deny your participation in services without your written consent, except in very limited situations

  • Your information cannot be shared with anyone—including employers, courts, law enforcement, or family—without your explicit written authorization, unless legally required

Disclosure without consent is permitted only in specific circumstances, such as:

  • A valid court order

  • A medical emergency

  • Mandated reporting of child, elder, or dependent adult abuse

  • An immediate threat of harm to yourself or others

  • Audits or program evaluations required by law

You may revoke your written consent to share information at any time, in writing, unless the disclosure has already occurred.

Your Rights

You have the right to:

  • Access and request a copy of your records

  • Request corrections to your information

  • Receive an accounting of authorized disclosures

  • Request restrictions or alternative communication methods

Telehealth & Digital Communication

If services are provided via telehealth or digital platforms:

  • Sessions are conducted using secure, HIPAA-compliant technology

  • You are encouraged to ensure privacy on your end during sessions

  • Email or text communication is limited to scheduling and non-clinical matters unless you authorize otherwise
